We have created a new Patient and Caregiver Advisory Council Nomination Certificate you can use to recognize a patient or caregiver you think would be a great addition to the PCI or Vascular Patient and Caregiver Advisory Council.

A new paper version of the interest and sign-up form for use by patients, caregivers, and health care teams is also available.

Sites that refer a joining council member can earn P4P extra credit.
