BMC2 and the Michigan Value Collaborative (MVC) have teamed up to form the new Michigan Cardiac Rehab (MiCR) Network. Despite the prominence of cardiovascular disease, too few patients and their families are familiar with cardiac rehabilitation, a life-changing program available to our cardiac patients. To help providers meet the new state-wide goal of 40% cardiac rehab utilization for TAVR, SAVR, CABG, PCI, and AMI patients, MiCR has created a new toolkit.

The kit outlines initiation, maintenance, and innovation strategies for increasing utilization of cardiac rehab and includes metrics. We invite you to turn to this tool often as you work to encourage the enrollment of more patients in cardiac rehab programs.
